Normally, melanin is pushed out and shed through the skin’s natural turnover process, maintaining an even skin tone. However, when the skin’s turnover is disrupted by accumulated UV damage or aging, the balance between melanin production and excretion is disrupted. This leads to excessive melanin accumulation in the epidermis, causing spots and freckles.

Freckles are often hereditary. They can also occur due to improper skincare. They are the most common concern among patients visiting us for pigmentation issues. Although termed “age spots,” they can appear as early as one’s 20s in some individuals. Generally, however, many people notice an increase around the age of 50.

They appear as brownish-black pigmented spots, often round or oval in shape, on areas frequently exposed to sunlight like the cheeks and temples. They respond well to laser treatment, making the results noticeable. In cosmetic dermatology, they are primarily treated with lasers or light-based therapies (IPL).

Melasma

Chloasma often develops in women in their 30s and beyond, particularly those who have experienced pregnancy and childbirth or who take birth control pills, suggesting a link to female hormones. Mental stress can also disrupt hormonal balance and contribute to its appearance. Additionally, excessive massage or physical irritation may worsen the condition.
A key characteristic of melasma is its symmetrical appearance on both sides of the face, typically with similar shapes and sizes. It often appears around the cheekbones, spreading in a relatively large, indistinct area with blurred edges.

Many patients troubled by melasma experience a pattern where, even after treating superficial spots, the underlying melanocytes—which produce the melanin causing the spots—remain highly active. This leads to a situation where no matter how much treatment is applied, the spots seem impossible to stop.
Strong laser stimulation can worsen the condition or cause latent melasma to surface. Therefore, extreme caution is required when using lasers for spot treatment. While other clinics may offer treatments like laser toning (multiple sessions with low-intensity lasers) or oral tranexamic acid, our clinic employs a fundamental treatment approach that minimizes skin stress and reduces the risk of recurrence.

Freckles (Ephelides)

These are small, brownish-tan spots, typically 1-several mm in size, scattered across the cheeks, under the eyes, and around the nose. They are more common in fair-skinned individuals and can appear from childhood, though they tend to increase around puberty. Due to hormonal influences, women are more likely to develop them than men. They are also a symptom influenced by genes that make the skin sensitive to UV rays, and cases worsening due to UV exposure or pregnancy have been observed.

Even if hereditary, they can improve with appropriate treatment. While treating regular age spots or melasma often takes priority, our clinic can simultaneously address freckles using IPL
light therapy, Needle RF (PW), and peels.

Post-Inflammatory Hyperpigmentation

This occurs due to skin damage from acne, wounds, or friction like scrubbing. When skin is damaged, melanocytes activate during the repair process, producing more melanin pigment than usual. If this melanin is overproduced, the skin’s normal metabolism cannot process it all, causing it to accumulate within the skin and appear as hyperpigmentation. It appears in various shades, such as grayish-brown, brownish-gray, or sometimes purplish-brown, and manifests locally in areas with a clear cause.

While it is generally believed that hyperpigmentation will naturally resolve within a year at most once the skin damage heals, depending on the cause of the pigmentation and individual skin type, it may take anywhere from several weeks to several years to fade. If it does not improve, it may indicate that the skin’s turnover cycle is not functioning normally, and treatment will be pursued.

Treatment Methods

Lumecca

Lumecca is a new light therapy (IPL) ideal for treating dark spots, dullness, freckles, skin rejuvenation, and improving skin texture.
